Health Connections' mission is to ensure Islanders are connected to support and information that enables them to live happier, healthier and more fulfilling lives. They have developed a 'Directory to Doorstep' model to achieve this which lists over 500 local community assets. The organisation trains Community Connectors who can signpost their friends, family, neighbour and colleagues to this support.
Their voluntary community transport service enables people to access these opportunities for social connection and health and wellbeing appointments and activities listed on the directory.
Health Connections provide one-to-one support and advocacy to people having treatment and care on and off-island, delivering community initiatives like the "Talking Cafe Project" allows people to meet with others and make new friends and talk with a Community Connector about support available in the community.
